CCMH houses two extensive physician groups. The Primary Care practices are integral to a large Federally Qualified Health Center, encompassing 22 primary care clinic locations. These clinics are strategically distributed, with some located on-campus, in-town, and extending into the surrounding catchment area. Within this group, there is a dedicated team of 75+ Family Medicine and Internal Medicine physicians and Advanced Practice Providers (APPs). Additionally, a centralized Referral Management Office plays a pivotal role in receiving and coordinating referrals, not only from CCMH's employed primary care providers but also from other healthcare providers in the community, ensuring efficient scheduling and prioritization for CCMH-employed specialty practices.
CCMH's employed Specialty Group operates centrally under the umbrella of the Memorial Medical Group, offering access to a comprehensive array of healthcare services through 17 different specialties, all conveniently located on the hospital's campus.
