  • Health, Dental, and Prescription Drug Plan – Plan 2 Medical Coverage and Dental Coverage for you. 
  • Term Life Insurance – employee coverage at 1 times annual earnings to a maximum of $250,000 (term life insurance flex coverage highlights attached) 
  • Short Term Disability - 70% of salary for 26 weeks (plan document attached) 
  • Long Term Disability – 60% of your monthly earnings to a maximum of $15,000

Retirement Savings (invested through the Bank of Oklahoma Finance BOKF)  

  • 457(b) – salary deferral in 2023 may not be more than $23,000 if you are under 50 years old; $30,500 if you are 50 years old or older and deducted bi-weekly through payroll and placed in your retirement account with BOKF. 
  • 401(a) CCHA Physician and Senior Management Retirement Plan – an amendment to your employment contract allows you to reduce your base salary by 10% for contribution to the 401(a) OR the maximum allowable of $69,000 that is placed in your retirement account with BOKF on a bi-weekly basis.
