Care teams
Your care team includes health professionals who combine their expertise in specific areas to provide you with the best possible plan for your health and wellness. They work together — and with you — empowering you to be the leader in your health care is one of their goals.

Your care team may include your primary care provider, a medical assistant, a registered nurse care manager, a pharmacist, a registered dietitian/certified diabetes educator, a psychiatric mental health practitioner and a mental health counselor. If you are interested in meeting with one of your care team members, ask your primary care provider.

Salem Health has two hospitals, two urgent care clinics, and primary care clinics throughout Marion and Polk counties. No matter where you live in the Mid-Willamette Valley, care is close to home.

100% Outpatient Primary Care 

  • 4-day work week (32 clinical hours)
  • Average 15-18 patients/day
  • Call – phone call only (1:7)
  • Epic EMR & Dragon Medical One Dictation Software
