Community Health Partners is the fastest-growing organization in Fresno County. We have opened 6 Primary Care clinics in the last 18-months and are looking to add two new ones in the upcoming months. Currently, Community Health Partners has 37 Primary Care providers including a newly opened OB office, 5 Pediatricians, and our Family Practice and Internal medicine outpatient providers. All clinics are in Fresno or Clovis and vary in size from 25 exam rooms and down. Community Health Plan is Fresno’s only locally based, commercial health plan.
