Primary Care

Community Health Partners is the fastest-growing organization in Fresno County. We have opened 6 Primary Care clinics in the last 18-months and are looking to add two new ones in the upcoming months. Currently, Community Health Partners has 37 Primary Care providers including a newly opened OB office, 5 Pediatricians, and our Family Practice and Internal medicine outpatient providers. All clinics are in Fresno or Clovis and vary in size from 25 exam rooms and down. Community Health Plan is Fresno’s only locally based, commercial health plan.    

We serve members based in Fresno and neighboring counties, providing access to a network of more than 2,100 providers, including 1,200+ physicians (primary care and specialists) and 400+ mental health practitioners, practicing at more than 700 sites and offering urgent care at 17+ locations.  Currently, we have 37 Primary Care providers that include Internal Medicine, Family Medicine, Pediatrics, and OBGYNs. Our private, not-for-profit healthcare network includes four hospitals and multiple outpatient clinics in the city. Additionally, we are the second-largest health plan in the state of California.    

Community Health Partners’ Family Medicine and Internal medicine doctors focus on preventative medicine and screening, chronic disease management, standard health, and wellness exams, as well as additional bread and butter elements of primary care. Our Family Medicine doctors see approximately 20 patients daily with the support of their own dedicated MA, Internal Medicine physicians will see 16 – 18 patients. Family Medicine doctors see PEDs to Geriatrics and Internal Medicine doctors see patients from 16 years old and above. This opportunity is 100% outpatient, M-F, 8 am to 5 pm, no nights or weekends.     

There are leadership opportunities available as well if you want to encompass that as part of your career path. Involvement in subcommittees, recruitment and vetting of new providers are available if you are looking to step into leadership.

All clinics and hospitals are linked with EPIC EMR, and we have Dragon Dictation for any providers that would like it. 

Call

You will be part of a call pool that is triaged prior to any calls going out to the providers by a nurse exchange group and will be shared equally amongst all the providers, NPs, and physicians. The exact call frequency will vary, as we hire more providers. Currently, the call group is growing and is above 1:20.

There are numerous indicators as to why there is a need for Primary Care in the greater Fresno area. Preventative Medicine and Chronic Disease management are desperately needed. The rate of infant mortality is nearly double in Fresno County as compared to the rest of the state of California. Furthermore, Fresno County tends to be worse than the state averages in asthma, obesity, teen births, and preterm deliveries.    

We pride ourselves on supporting each physician’s vision for their own practice. To help with continuity of care for our patients the Internal Medicine doctors will be co-located together.
