Specialty Services: Integrated Position

This LCSW/LMSW works in the family medicine clinics alongside the providers (FPOB, FP, PA, FNP) and nurses. When the provider sees a patient or family that could use additional support or connection to services, the provider will bring the LCSW in to work with the patient/family. This can be done in conjunction with that visit or by scheduling an additional visit. (Usually 15-30 minutes) They can also have a schedule of their own with follow up visits to make sure the patient is thriving. The LCSW matches resources and solves problems that can be handled in a relatively short-term period. (Referrals can be made to therapists for those patients requiring long term and ongoing therapy.) They can see patients from cradle to grave and includes entire spectrum of issues like diabetes, weight, mental health, anxiety, depression, etc. Must be flexible and adaptable as every day can be different with changes to the schedule and new patients. Summer seasons tend to be less busy, and it gets busier during the winter. Candidate needs to be flexible, reliable, visible (working in open space). The Burley location will be more fast paced, and candidate must be able to accommodate 2 types of clients which are the patient and the doctor. LCSW is providing clinical services with a focus on behavior and specific change and targeted brief interventions.
