Employment agreement between Northwest Michigan Health Services, Inc., a 501c3 nonprofit corporation located in Traverse City, Michigan (Employer) and Provider.
Reimbursement of cost or licensure and DEA renewal. Reimbursement for one annual membership up to $500.
Relocation and sign-on bonus to be included in first payroll.
During the first ninety (90) days of the term of the agreement, either party may terminate the agreement without cause upon thirty (30) days written notice to the other party. After the first ninety (90) days of the term of the agreement, either party may terminate the agreement without cause upon forty-five (45) days written notice to the other party.
NMHSI shall obtain and maintain professional liability coverage through the Federal Tort Claims Act during the term of the agreement. NMHSI is not responsible to procure professional liability insurance for any past or future acts or omissions.
Up to $50,000 for Student Loan Repayment Program via National Health Service Corporation.
Fringe benefits include without limitation, comprehensive health insurance, vision, dental, life and accidental death and dismemberment, short-term disability, and long-term disability insurance. 401k retirement account, paid holidays, and 128 hours of annual paid time off (PTO). NMHSI agrees to provide Practitioner four (4) paid days of educational leave to be used for CME and reimbursement of related expenses up to a total of $2,000 for each year during the term of the agreement. The practitioner will receive 128 hours of annual paid time off which is front loaded, not accrued. Practitioner may carry over a maximum of 40 hours of unused PTO from one year to the next year, but subject to a cap of 200 hours of PTO. All of the fringe benefits are provided at the sole discretion of NMHSI, and benefits may be added, deleted or modified, as NMHSI determines to be in its best interests.
The outpatient practice schedule is comprised of 40 hours of patient contact, with NO CALL, NO WEEKENDS and NO MAJOR HOLIDAYS!!
Once a candidate’s submittal dossier is submitted to Mindy Aguilar, HR Director, a screening call will be completed between candidate and Mindy.
The Benzonia location is currently staffed with (2) registered dental hygienists.
