Understanding the Role

San Antonio Regional Hospital is recruiting a full-time Internal Medicine physician to serve as the lead clinician and resident preceptor at its new Center of Aging in Rancho Cucamonga, California. This is an outpatient-only position with no inpatient responsibilities, no call, and no overnight requirements. The Center of Aging is located approximately three miles from the main SARH campus and is currently in a pre-opening phase, with a target launch in 2026 tied to the incoming Internal Medicine resident cohort.

The physician will function as the clinical and educational anchor of the new center, supervising a team of Internal Medicine residents while overseeing day-to-day clinic operations. The position is structured as a 1099 independent contractor arrangement through Premier Medical Group, the private group managing the clinic under the SARH umbrella.

Role and Responsibilities

The physician's primary responsibility is the clinical supervision and education of Internal Medicine residents rotating through the Center for Aging. The position is structured around a preceptor model, meaning the physician will oversee resident-patient encounters rather than managing an independent patient panel.

  • Supervise 2 to 4 Internal Medicine residents during clinic hours
  • Provide direct supervision for first-year residents and indirect supervision for second- and third-year residents as their confidence develops
  • Precept resident-patient encounters, reviewing cases, answering clinical questions, and stepping in when needed
  • Oversee one Nurse Practitioner working four days per week
  • Manage a limited number of direct patient encounters when resident coverage gaps occur (typically no more than five per day)
  • Handle administrative tasks and inbox management using Friday afternoon admin time
  • Contribute to the academic structure of the Internal Medicine residency program at SARH

Qualifications

Required:

  • MD or DO degree
  • Board-certified or board-eligible in Internal Medicine
  • Eligible for medical licensure in the State of California
  • Comfortable working in an academic-clinical hybrid environment

Preferred:

  • Interest in graduate medical education and resident teaching
  • Experience or familiarity with outpatient geriatric and post-discharge patient populations
  • Adaptable, solution-focused approach to a growing, early-stage clinic
  • Business awareness appropriate for 1099 independent contractor practice structure

Reporting Structure

The physician reports directly to Dr. Bilal Rayes, Associate Program Director for the SARH Internal Medicine Residency, and Dr. Sara Khan, who oversees clinic operations. A dedicated office manager is on-site for day-to-day administrative support.

Credentialing for this position is clinic-based only, not hospital credentialing, which significantly shortens the typical onboarding timeline.
