Understanding the Clinical Component

Located one block from Wyoming Medical Center, the WCPS office provides a comprehensive suite of in-office procedures, ensuring that you have everything needed for optimal patient care. With our accredited nuclear medicine and echo/ultrasound systems, and support from a professional team of APPs who assist with patient follow-ups and inpatient care, you are well-equipped to build a successful Interventional Cardiology practice. There are 10 outreach clinics that service two-thirds of the state. The EMR used at the clinic is Allscripts and the EMR used at the hospital is Cerner.

There will be a ramp-up period of about 6-months. The beginning stages of the practice are going to be on the lighter side while the provider is integrated into the community and builds a patient panel. Having the flexibility to offer some general cardiology services while ramping up will assist with referrals as this is a smaller community and word of mouth travels quickly. Providing any additional cardiology services will also help the salary of the provider during the first year as they build up their patient panel.

The office is open Monday through Friday, 8am to 5pm. This position can expect to see 20-25 patients per day in clinic. The basic schedule will be:
- 2 weeks of clinic & 1 week of inpatient/call.
During the 2 weeks in the clinic, you will be able to be as busy as you would like and have time to do any office-based procedures you would like.

The week you are doing inpatient/call, there is an average of 5-8 patients that need to be rounded on daily, this will include both General Cardiology and Interventional Cardiology patients. You will also do 2-4 Interventional Cardiology inpatient procedures & as many outpatient procedures as time allows. Most providers will also schedule 2-4 PCIs on Saturday and 1-2 on Sunday.

On an annual basis, providers are doing about 200 PCIs & 75 PVs per year in addition to the group handling 25-30 Impella’s per week.   

Work Schedule:

  • Two weeks of clinic work, with the expectation to see a high volume of outpatient cases.
    1-2 Days a week doing outreach
  • One week covering the hospital, including:
    Managing 5-8 inpatients and performing 2-4 inpatient procedures along with outpatient procedures.
  • One full day in the cath lab during the week before your on-call week. 

You will have the privilege to work alongside four wonderful APPs: Chelsea, Stephanie, Rachel, and Arianne. We had the opportunity to meet them personally and they are sharp. They work independently but are true extenders of the group. Their role is to provide follow-up and support for the cardiologists. They do not have their own patients, but they do have practice autonomy. The cardiologists trust them. They will often field most of the phone calls from the nurses. They monitor nuclear tests, see patients in clinic, do telemedicine, help on inpatient rounds, and supervise stress tests, handle hospital discharges, among other duties. The APPs also work on Saturday and Sunday from 8am-Noon helping with discharges, rounding and stress tests.  

Outreach:  
Outreach efforts include traveling for up to two hours to nearby locations (Douglas, Buffalo, Lander, Riverton, Rawlings) 1-2 times a week during clinic weeks. This is essential for maintaining strong community relationships and ensuring that patients in more remote areas have access to high-quality cardiology care. All of the Cardiologist do this outreach and the specific number of outreach days is variable based on need.  

Structural Heart: 
Both Dr. Fluture and Dr. Idelchik do Structural Heart cases, TAVR & Watchman so there would be no need for another provider to perform these cases.

Hospital Facilities:  
Banner Wyoming Medical Center is the largest hospital in Wyoming with two campuses and 14 primary, specialty and immediate care clinics. The Banner Wyoming Medical Center has a total of 249 beds across two campuses. This makes it the largest hospital in Wyoming, providing comprehensive medical services and serving as a regional trauma and referral center for 11 Wyoming counties. 

Banner Wyoming Medical Center offers comprehensive cardiology and heart services aimed at providing top tier care for a variety of heart conditions. The center's services include advanced diagnostic and therapeutic procedures performed in their state-of-the-art cath labs, such as cardiac catheterizations, angiograms, pacemaker implantations, and treatments for stenotic heart valves. Additionally, the center performs cutting-edge procedures like the WATCHMAN heart implant, Transcatheter Aortic Valve Replacement and soon Mitraclip.  

The practice has a state-of- the-art nuclear medicine system (accredited) and echo/ultrasound system (accredited). This position allows for immediate patient volume uptake with the existing demand. The hospital offers two state-of-the-art cath labs, and an active open-heart surgery program with two board certified cardio-thoracic surgeons. Additionally, there is a hybrid room that is currently being used 1 day, every other week for Structural Heart procedures and a dedicated space for a new EP lab.   

The hospital partners with several specialists to deliver these services, including a highly experienced cardiology support team. Notably, Dr. Adrian Fluture serves as the medical director of cardiovascular services and plays a significant role in performing advanced cardiac procedures.

The cardiology department provides a broad spectrum of services, including: 
Diagnostic Tests: Cardiac catheterization, exercise stress tests, electrocardiograms (EKGs), echocardiograms, chest Xrays, and MRIs. 

Treatments: Angioplasty (PCI), coronary artery bypass surgery, and percutaneous closures of patent foramen ovale and atrial septal defects. 

The hospital features: 
Therapeutic Services: Angioplasty (Percutaneous Coronary Intervention - PCI), Coronary Artery Bypass Surgery, WATCHMAN Device Implantation, Transcatheter Aortic Valve Replacement (TAVR), Percutaneous Closure of Patent Foramen Ovale and Atrial Septal Defects, Pacemaker Implantation.  

Procedures: Angiograms, closure of congenital heart defects, treatment of stenotic heart valves, and pacemaker implantations are performed here, typically under local anesthesia. 

State-of-the-Art Facilities:  
Two State-of-the-Art Single Plane Units: Equipped with diagnostic imaging equipment for catheterization procedures. One unit provides three-dimensional views of the coronary arteries, enhancing diagnostic accuracy and procedural efficiency. 

Hybrid Suite: Combines the capabilities of a traditional operating room with advanced imaging equipment, allowing for a variety of complex procedures to be performed with greater precision and flexibility. 

Incoming Electrophysiology (EP) Lab: Equipped with cutting-edge technology for diagnosing and treating heart rhythm disorders. 

Experienced Cardiology Support Team: The team includes highly trained nurses, technicians, and other support staff who ensure comprehensive care for all cardiac patients. 

Accredited Systems: 
Nuclear Medicine: Uses small amounts of radioactive materials to diagnose and determine the severity of heart disease. 

Echo-Vascular Ultrasound Systems: Non-invasive tests that provide detailed images of the heart and blood vessels.
