Position Overview

Understanding the Role

Building the Future of Geriatric Care in the Shenandoah Valley

Imagine joining a forward-thinking team at the ground level of an ambitious geriatric medicine program, where your expertise will directly shape the care model for an entire region's aging population. As the incoming geriatrician at Augusta Health in Fishersville, Virginia, you'll step into a role that combines the intimacy of relationship-based medicine with the excitement of program development. This isn't just another geriatric position – it's an opportunity to be a founding member of what will become the Shenandoah Valley's premier transitional care team, with plans already underway to establish a PACE (Program of All-Inclusive Care for the Elderly) facility within the next five years.

Your practice will serve the growing senior population across a manageable 15-mile radius around Fishersville, where Northern Virginia retirees are increasingly choosing to spend their golden years amid the natural beauty of the Shenandoah Valley. Working alongside Dr. Kokanovich, the recently appointed Medical Director who arrived from Arizona just last November with a vision for transformative geriatric care, you'll help build a program that addresses the full spectrum of elderly care needs – from routine chronic disease management to complex behavioral health challenges. The community's need for specialized geriatric expertise is significant and growing, with eight assisted living facilities currently relying on a single physician assistant to manage care for over 240 residents, with immediate plans to expand the patient panel to 320.

  • Board-certified Internal Medicine physician with completed Geriatric Medicine fellowship required – recent graduates particularly welcome for their fresh training in interdisciplinary care models and behavioral health integration
  • Full-time position with flexible four-day work week option – typically four 10-hour days with choice of weekday off (Monday currently available)
  • New position expanding existing team – not a replacement, but true growth reflecting increasing community need and strategic vision
  • Direct reporting to Dr. Kokanovich, Medical Director of Transitional Care, with strong administrative support from Bobby Coffey, Office Manager (25 years with Augusta Health) and Rhonda, Physician Recruiter
  • Academic opportunity available – potential teaching role in Augusta Health's Internal Medicine residency program and planned psychiatric residency program
  • Collaborative leadership role in developing the region's first PACE facility targeted for the Charlottesville area, positioning you at the forefront of innovative elderly care delivery
  • Unique market position as Augusta Health's strategic answer to regional competition from Adventist Healthcare, with the selling point of seamless care coordination when residents transition between facilities
  • Comfort with psychiatric illness essential – two of the eight facilities house residents with mental health issues alongside their medical comorbidities

This role represents more than a job opportunity – it's an invitation to become an architect of comprehensive geriatric care in a region experiencing significant demographic transformation, where your clinical expertise and vision for interdisciplinary care will directly impact hundreds of lives while building something lasting for the community.
