You'll join a refreshingly stable team where longevity speaks volumes about job satisfaction and organizational culture. Your closest collaborator will be Ken Donahue, PA-C, a dedicated physician assistant who has spent the past seven years building relationships with residents across six assisted living facilities, creating a foundation of trust and continuity that you'll build upon together. Ken's deep institutional knowledge and established rapport with facilities makes him an invaluable partner as you expand the program's reach. Working his preferred schedule of four 10-hour days (off Mondays), Ken has perfected the art of balancing direct patient care with the extensive coordination work that defines excellence in geriatric medicine, and he's eager to share the patient panel with a physician partner who values collaborative practice.

At the helm of this growing program, Dr. Timothy Kokanovich brings fresh energy and vision, having relocated from Arizona just last November specifically for this leadership opportunity – drawn by family connections and the chance to raise his family in a community with exceptional schools. His enthusiasm for building something meaningful is palpable, and he speaks passionately about the absence of burnout in this practice model, crediting the variety of cases and deep patient relationships for keeping the work engaging and sustainable. Supporting the clinical team, you'll work with Laura, CMA, a certified medical assistant who serves as both a skilled historian and phlebotomist, conducting initial assessments that allow you to focus on complex medical decision-making.

  • Ken Donahue, PA-C – 7-year veteran covering six facilities, experienced in all aspects of geriatric and transitional care, maintains four-day schedule seeing 5-10 patients daily
  • Dr. Timothy Kokanovich, Medical Director – Recently arrived from Arizona (November), leads both transitional care and palliative/hospice services, passionate about team-based care and PACE program development
  • Bio for Dr. Kokanovich: Bachelor of Science in Medicinal Biochemistry from Arizona State University; MD from St. George’s University; Internal Medicine residency at Loma Linda University; Fellowship in Hospice and Palliative Medicine at Mayo Clinic Arizona. Formerly practiced with Hospice of the Valley and Mayo Clinic in Arizona. Relocated with family to Shenandoah Valley in November 2024. Now Medical Director of Palliative Care and Transitional Care at Augusta Health, leading a multidisciplinary team across multiple care settings. Associate Hospice Medical Director for Hospice of the Shenandoah. Passionate about strengthening community-based care and advancing Augusta Health’s mission.
  • Laura, CMA – Dual-trained as medical assistant and phlebotomist, conducts preliminary histories and physicals, manages vital signs and blood draws
  • Bobby Coffey, Office Manager – 25-year Augusta Health veteran providing exceptional administrative stability and institutional knowledge
  • Rhonda, Physician Recruiter – Works closely with Dawn Johnson, Manager of Provider Recruitment, ensuring smooth onboarding and ongoing support
