This Family Medicine position provides an exceptional opportunity to practice patient-focused, comprehensive care in a modern clinic environment. The clinic serves a diverse patient base, primarily employees of Fieldale Farms and their families, spanning all age groups from two years and up. The patient population reflects the cultural richness of the community, with approximately 60% of patients speaking Spanish and additional representation from languages such as Hindi, Vietnamese, Gujarati, and Laotian.

Patient Demographics and Services
The clinic emphasizes chronic disease management, with a particular focus on diabetes and hypertension, conditions prevalent in the workforce population. Preventative care is another cornerstone of the clinic’s mission, offering wellness visits and screenings that many patients may not have previously accessed due to systemic barriers. Acute care is also provided, addressing urgent health needs to reduce reliance on emergency departments and minimize downtime for employees.

The majority of visits are scheduled for 30 minutes, with extended time allocated for wellness visits involving non-English-speaking patients to ensure comprehensive care. Providers are supported in managing complex cases through an integrated approach that combines clinical expertise with community-focused solutions.

On-Site Resources and Technology
The clinic is equipped with state-of-the-art facilities to enhance patient care. Key resources include:
• Digital Radiology and Lab Services: Patients benefit from immediate access to X-rays and on-site laboratory testing. A digital radiology service allows rapid reads by radiologists, typically within 15–20 minutes.
• On-Site Full-Service Walgreens Pharmacy: Open from 9 AM to 6 PM, the pharmacy provides cost-effective prescription options, including waived co-pays for participants in diabetes management programs. Pharmacists also assist with medication education and prior authorizations, easing the administrative burden on providers.
• Epic EMR System: Seamlessly integrates with local hospitals and specialists, ensuring continuity of care for patients requiring referrals.

Specialized Care and Programs
The clinic takes a proactive approach to chronic disease management, particularly diabetes. A PharmD-certified diabetes educator visits weekly to offer specialized education and support. Patients who engage with the diabetes program gain access to waived co-pays for medications and glucose monitors, significantly improving adherence and outcomes. This initiative reflects the clinic’s commitment to incentivizing wellness and reducing barriers to care.

Opportunities for procedural work are available, with providers credentialed by Premise Health to perform an array of office-based procedures. These are typically conducted in the clinic’s fully equipped exam rooms, streamlining care delivery and minimizing patient referrals.

Collaborative Support Structure
Providers work within a collaborative, team-based environment designed to enhance efficiency and quality of care. The clinical team includes:
• Medical Assistants (MAs): Fully trained in phlebotomy and bilingual in Spanish to support patient interactions and lab draws.
• Bilingual Receptionist and Practice Administrator: Ensure smooth clinic operations and effective patient communication.
• Nurse Practitioners (NPs) and RNs: The clinic is expanding to include an RN supervisor and a second provider to maintain manageable patient volumes while accommodating growth.

The clinic’s operational model emphasizes work-life balance, with a fixed schedule and no after-hours or weekend responsibilities. Providers can focus entirely on delivering exceptional care without the pressure of overbooked schedules or call duties.

Culturally Competent Care
Recognizing the diverse cultural and linguistic needs of its patients, the clinic prioritizes hiring bilingual staff and providers. It employs interpreter tablets for non-Spanish languages, although the use of bilingual providers is considered essential for fostering trust and rapport with the community. Providers with an interest in cultural competency and patient engagement will find this role both challenging and rewarding.

Vision for Growth
As the clinic continues to expand, the goal is to establish a two-provider model, with each provider managing a patient load of 12–14 visits per day. This structure ensures high-quality care while allowing flexibility to accommodate walk-ins and seasonal increases, such as during flu season. The clinic’s leadership is committed to maintaining a balance between growth and the personalized care that defines its mission.
