As a Hospitalist Advanced Practice Provider (APP), you will play a vital role in delivering high-quality inpatient care within a Critical Access Hospital setting. Working alongside a dedicated hospitalist physician, you will independently manage a daily patient panel of approximately 12 patients, with 1-3 new admissions per shift. While you will have autonomy in patient management, you will also benefit from real-time physician collaboration and a strong multidisciplinary team that meets daily for 10 AM rounds. This structured approach ensures seamless communication among physicians, APPs, nurses, case management, pharmacy, and therapy staff, allowing for a well-coordinated care plan for each patient.
This hospitalist role is designed for APPs who thrive in a broad-scope inpatient environment. While the hospital includes a 6-bed ICU, APPs focus exclusively on floor and observation patients, as hospitalists manage all ICU-level care. There are no procedural requirements for APPs—intubations, central lines, and other advanced interventions are performed by hospitalists, emergency physicians, or anesthesia staff. With limited on-site specialist availability, you will develop strong clinical decision-making skills, utilizing remote consults for cardiology and oncology, while collaborating closely with on-site surgical specialists, including orthopedics, general surgery, urology, and neurosurgery.
The hospital operates under Critical Access guidelines, meaning the total inpatient census is capped at 25 patients, with additional swing bed and observation patients. There is not another critical access hospital within 35 miles and all patients that can’t pay, will be reimbursed at cost of providing care. Due to the 96-hour rule, patients requiring extended care are typically transferred to Billings, MT (90 miles away). Fortunately, case management and a well-established referral network streamline these transitions, ensuring patients receive appropriate care without unnecessary delays.
From a workflow perspective, the 7-on/7-off schedule provides both predictability and work-life balance, with shifts running 6 AM - 4 PM on weekdays and 6 AM - 2 PM on weekends. Additionally, telemedicine services provide overnight coverage, eliminating the need for overnight call responsibilities. The hospital utilizes EPIC EMR, though it is integrated with Intermountain Health’s system in Billings, requiring some workflow adaptations.
