Healthcare System Overview

Delivering Quality Care in a Critical Access Setting

As a Critical Access Hospital (CAH), this facility is dedicated to providing high-quality, patient-centered care while maintaining a low patient-to-provider ratio. With a 25-bed inpatient capacity, the hospital ensures that each patient receives personalized attention, while also efficiently coordinating higher-acuity transfers to regional partners in Billings, MT. The hospitalist program, now fully in-house, has been restructured to offer stability, provider satisfaction, and seamless patient continuity.

Hospitalist Program & Inpatient Care

The hospitalist program was previously managed by Sound Physicians but transitioned to an in-house model to enhance provider retention and operational consistency. The team is structured to maintain three full-time hospitalist physicians and two full-time APPs, ensuring balanced coverage. The one physician–one APP model during daytime shifts allows for strong collaboration, while telemedicine providers handle overnight coverage, eliminating overnight call responsibilities.

Facilities & Clinical Services
• 25-Bed Capacity – As a Critical Access Hospital, inpatient stays are subject to the 96-hour rule, with a well-coordinated system for transfers to Billings, MT (90 miles away).
• 6-Bed ICU – The ICU is managed exclusively by hospitalists, with APPs focusing on floor and observation patients. While some ICU-level care is provided (e.g., BiPAP, high-flow oxygen, AFib with RVR on drips), patients requiring prolonged critical care, intubation, or advanced interventions are transferred.
• Emergency Department – Staffed by board-certified emergency physicians who play a key role in triaging and determining admissions vs. transfers.
• Surgical Services – A robust surgical department includes general surgery, orthopedics, urology, neurosurgery, and anesthesia, ensuring a wide range of inpatient and outpatient procedures.
• Limited On-Site Specialty Support – Cardiology and oncology consults are available remotely, with strong referral partnerships for specialty care.

Technology & Resources
• EPIC EMR System – The hospital operates on a version of EPIC that is integrated with Intermountain Health, requiring occasional workflow adaptations.
• Telemedicine Services – Overnight patient management is handled remotely, ensuring seamless 24/7 inpatient care without additional provider burden.

Referral & Transfer Network
• Efficient Transfer System – The hospital partners with Billings Clinic and Intermountain Health to ensure timely patient transfers for higher-acuity care.
• Smooth Coordination – Most transfers occur without major delays, with the longest reported wait time being 12 hours.

Commitment to a Positive Work Environment
This hospital fosters a tight-knit, collegial environment, where providers, nurses, and support staff work closely together to prioritize patient outcomes and provider well-being. Administrative leadership is actively involved in ensuring work-life balance, fostering a culture of teamwork, stability, and long-term retention.
