Who We Are  

The mission of Project H.O.P.E. is to improve the health and well-being of homeless persons and others in need within the greater Camden, New Jersey area by providing primary, preventative, and related health services. 

Project H.O.P.E., Inc. (Homeless Outreach Program Enrichment) serves the medical and social needs of the homeless population in Camden County, particularly Camden City. The City of Camden is the largest urban center in southern New Jersey, with a population of 78,675 residents, and is ranked as the most economically depressed city in New Jersey and one of the most economically depressed cities in the United States. 

Project H.O.P.E. has fulfilled its mission by assisting more than 25,000 homeless persons in their journey to permanent housing and self-sufficiency. Project H.O.P.E. is unique in that it is the only provider of medical services specifically for the homeless in Camden County. 

Project H.O.P.E. currently operates a full-time health center providing culturally sensitive comprehensive primary health care serving patients at three locations: its main site, West Street Health Center, a satellite center at the Camden City Volunteers of America site, and through its Mobile Health Van which takes health care, mental health and substance abuse services to the streets and points where homeless people gather in Camden. 

In 2020, Project H.O.P.E. fulfilled a total of 15,089 medical and behavioral health visits for 3,442 unduplicated patients. A closer look at our patient population shows the significant barriers they face: 88% of patients were at or below the Federal Poverty Line, and 13% were without any form of health insurance. The three most common diagnoses are diabetes, hypertension, and depression. This reflects a high occurrence of chronic conditions within a patient population that has significant barriers to care due to lack of insurance or income. In addition, being impoverished creates a challenge when trying to access other things associated with healthy lifestyles, such as a proper diet or exercise. 

As COVID-19 emerged in the United States in early 2020, is classified by the World Health Organization (WHO) and the Center for Disease Control (CDC) as a global pandemic, Project H.O.P.E., Inc. expanded its services to its community. Over 6,500 COVID-19 tests were conducted in response to a statewide effort to stem the growth of this deadly virus. As COVID-19 vaccines were approved under Emergency Use Authorization (EUA), Project H.O.P.E., Inc. has administered over 4,000 COVID-19 vaccinations (as of 3/5/21). 

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.  

Medical Home

Project H.O.P.E. is recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home. 

Patient-Centered Medical Home (PCMH) is an approach to care that focuses on care coordination and communication between the patient and the medical provider. 

Project H.O.P.E. strives to provide care in a way that meets patients’ needs when and where they need it, in a manner that is accessible. 

Services 

Project H.O.P.E. provides primary and preventive care via a Team-Based Care model. Your care team–consisting of a medical provider (Nurse Practitioner or Physician), Nurse, and Medical Assistant–will work collaboratively with you to provide comprehensive medical care.

Consumer Advisory Board

As a patient of Project H.O.P.E., you can be involved with the Consumer Advisory Board (CAB), which meets monthly. 

The Consumer Advisory Board (CAB) is made up of current or former patients working together with Project H.O.P.E. to provide outreach to the community. They also advise Project H.O.P.E.’s Leadership Team on patient perspective in response to the health center’s services, initiatives, and outreach. 

Because of the advice gathered from the Consumer Advisory Board (CAB), Project H.O.P.E. can evolve with the needs and concerns of the community we serve and fulfill our mission of providing high-quality health care to the homeless and at-risk community.
