The base salary for this position is going to be based on experience and has a range between $300k-$400k and this would be a guarantee for the first year and if the provider stays there would be negotiations based on the market for an increase in salary.
Sign-on: Percentage of salary portion up front, portion 6 months in and a portion as more of a retention bonus for a second year of service. (Example: $45k, it would be $15k, then $10k, then $20k)
Relocation: $10k (negotiable upon their needs)
CME: $5k and 5 days off
Licensure: $4500
Weeknight call: $200-night
